For Those with Insurance and For Those Without

At Ballard Neighborhood Doctors we are committed to providing care for all patients regardless of insurance or ability to pay.  We thus accept most private insurers, Medicare and Medicaid/ Apple Health.  We also offer a generous sliding scale for those who need it.  And for our established patients, we do have a medical scholarship fund to help when times are particularly tight.

Uninsured

Non contracted insurance patients

Medicare patients

If you are uninsured or have insurance with whom we are not contracted, we offer discounts for payment at the time of service.  For all Medicare patients, to receive care at our clnic you must fill out a time of service payment form for instances when you may need to see Dr Bell.  As a naturopath he can not bill Medicare but we must be able to see you for urgent needs and when Dr Weakland is on vacation.  Please ask us for the time of service discount form.

Insurance

We are not accepting new patients with Cigna or Aetna insurance given poor reimbursement contracts and lack of interest in working with us to provide quality care.  We are contracted with these groups and will continue to care for our existing patients who transition to Cigna or Aetna but do ask that you consider other options when possible.

For similar reasons we no longer are contracted with United Community Health Plan (Apple Health).  We ask all of our Medicaid/ Apple Health patients to please choose Molina as your provider.

We do no currently accept United Med Advantage Medicare also based on poor experiences in working with them.

We are contracted with the following insurers:

Blue Cross Blue Shield (verification of coverage for both providers is highly recommended!)

First Choice Health Network - including MODA Health and ASSURANT Health

Group Health - Access plans only (verification of coverage for both provider is highly recommended!)

Lifewise of WA; Lifewise of OR

Molina

You must clarify and specifically request Molina as your provider and you must designate one of our providers as your PCP.

Please call your insurance company and use our Insurance Worksheet to help you understand your coverage.

Please know that insurances often have different coverage for ND and MD visits. We can not guarantee which doctor will see you,so please be prepared for billing to either physician.  When necessary, if your insurance does not cover a visit you will have to pay out of pocket for the services provided.

While we are happy to assist you, we can not be responsible for the many nuances within the wide variety of plans.  We can not know the specifics of your individual coverage.  Again, please use our Insurance Worksheet to assist you in verifying your own coverage.

It is your responsibility to confirm with your insurance company that they will cover services at our clinic by each of our providers.  You will be responsible for any charges not covered by your insurance.  We can not guarantee that both of our doctors are considered "in network" for your particular plan nor that the services you receive are a covered benefit within your specific plan.  Many plans now also have large deductibles meaning you will be responsible for your bill.  It is helpful for you to know your plan details to avoid surprises and stress after being seen.  Insurance dictates the rules of medical billing and payment.  By contracting with insurance companies we must agree to their rules.

Our Mission:  provide the finest level of primary care built on relationships of respect and trust.
